A program slot is not considered reserved until the enrollment deposit and “Intent to Enroll” form are received.
Before you attend Adelphi’s Pre-College Program, return the following completed forms to the Office of Summer and Intersession Programs (by mail or email).
- Health History and Physical Examination Form
- Immunization Record
- Residence Hall Agreement
- Acknowledgement of Program Policies and Procedures
- Acknowledgement of Risk and Release and Waiver of Liability Form

Effective May 11, 2023, Adelphi University will no longer require students, faculty, and staff to be vaccinated against COVID-19. Initial series and boosters are recommended (not required) for all students and employees in accordance with CDC guidance. Summer Pre-College Program students are not required to be vaccinated and/or boosted against Covid-19.
